APPROVAL OF THE CONTRACT BETWEEN JEWISH FAMILY & CHILDREN'S SERVICES OF SARASOTA-MANATEE, INC. AND THE SCHOOL BOARD OF SARASOTA COUNTY, FLORIDA
Description
This contract provides case management services to Cyesis teen parents, their children and their families, including assistance with the Medicaid process and home visits.
Services will be provided three days per week from January 11, 2010 through May 7, 2010. Payment shall be made in five equal payments of $1,541.40 starting February 28, 2010.
Board approval is necessary since the amount of this contract surpasses the annual minimum limit for this vendor.
Recommendation
That the contract between Jewish Family & Children's Services of Sarasota-Manatee, Inc. and the School Board of Sarasota County, Florida, be approved.
